Many authors think they can get a head start on the index by collecting entries while I do the camera ready work. This, of course, is up to them. However, it is harder for an indexer to find your words than to just read the manuscript and use their expertise to choose entries. And, if you are going to do the index yourself, you may find that it is easier to choose the entry and be able to write down the page number immediately, than to find out where the entry is on the final galley proofs. "Mt. Everest" may have been on page 13 of your draft, and page 16 on one of my early camera ready drafts, but on the camera ready galley proofs it may be on page 19.
